- Do NOT staple them together
- Must be completed in black/blue ink or you can type in your responses. Applications written in pencil or colored pen will NOT be accepted.
- When entering dates, you must use the Austrian date format (DD-MM-YYYY)
- The column on the right-hand side will be completed by the Austrian Consulate, so you can leave it blank.

The Austria visa application is not numbered, so it is crucial that you verify these directions as you complete your application form.
1. Enter your last name
2. Leave BLANK, unless you have a maiden name
3. Your first name and middle name (if you have one)
4. DD/MM/YYYY of your birth date
5. Enter where you were born
6. Country where you were born
7. Enter your current nationality, also include if you were born with a different nationality
8. Select your sex
9. Most students will choose Single
10. Unless you are under 18 years old, leave this BLANK
11. If you have studied in Austria before or have an Austrian identity card number, leave this BLANK
12. Choose Ordinary Passport
13. Enter your passport number
14. DD/MM/YYYY of when your passport was issued
15. DD/MM/YYYY of when your passport will expire
16. Country that issued your passport (e.g. US Department of State)
17. Enter your home address and email address, also include your phone number
18. If you are living in your country of citizenship, leave this BLANK. If you are a citizen of another country and applying at a consulate in a country where you only have your residence, then you need to put your passport number of your home country. If you have multiple nationalities but are applying to a consulate in a country where you are a citizen, then you can leave this BLANK
